The Heart Function Clinic is a specialized interdisciplinary team that provides follow-up care and education to out-patients with Congestive Heart Failure that are followed by a Cardiologist of Trillium Health Partners..
In the Heart Function Clinic patients with heart failure and their caregiver(s) collaborate with the interdisciplinary care team to create a tailored self-management program. The goal is to enhance the patients and caregiver's skills and confidence so that they can be actively involved in their own care. The Heart Function Clinic has proven to be effective in increasing the quality of life for patients and decreasing their need to use acute care services.
Specific Services Offered
- Creation of an individualized Person Centred Comprehensive care plan to manage Heart Failure
- Individual consultations with a Nurse Practitioner or a heart doctor (Cardiologist) and a Registered Nurse on each visit
- Individual nutrition counselling with a Registered Dietitian
- Individual medication counselling with a pharmacist
- Individual and group virtual education
- Access to cardiac rehabilitation program

Please note that due to the high number of patient visits to our hospital every day, you may experience clinic wait times and a delay in finding parking. We encourage you to arrive early and allow for extra time on the day of your appointment. If you arrive more than 15 minutes late, the appointment will be rescheduled to another day.
