Outpatient Services

THP has an outpatient Palliative Care clinic located at Credit Valley Hospital on the 3rd floor, Ambulatory Care area.

We will provide consultation and care on anyone with a life limiting illness needing an extra layer of support and assessment. We also provide home visiting services in select areas.​


Palliative Referrals

How & why did I get referred?

One of your doctors feels you require an assessment around your symptoms, goals of care, and to plan around your illness.

We will work alongside or provide consultation to your family doctor, medical specialists and the home care team.

What happens once I am referred

A Palliative Care nurse navigator will contact you to explain what to expect, and ensure you are receiving the right care in the right place and the right time.

Who is a part of the team?

  • We are a group of 17 physicians. You will be assigned one main doctor, but may encounter other team members at various points in your care
  • We work with a team of nurses in the outpatient clinic who will answer your phone calls, questions, concerns, and provide ongoing support for your needs
  • Our administrative assistants will coordinate, and guide you to appointments
  • We engage the help of other team members as needed, eg. Dieticians, social workers
  • We work very closely with the palliative home care partners through Home and Community Care Support Services as well as our Hospice colleagues

Getting an appointment:

Your doctor needs to make a referral. A specialized Palliative Care nurse navigator will decide what provider will match your needs. If our team is the best fit, then you will be called for an appointment.

What to expect?

  • the first appointment is booked for an hour.
  • We review your understanding around your illness
  • We review your symptoms and medications
  • We review your goals/values/wishes, and anything else that can be done to make your day better.

What do I need?

  • Health card
  • all your medications
  • A support person who you want to share your health information and wishes with
  • any health documents eg. Power of attorney, living will

Home Visits

How do I get a palliative doctor to visit my home?

We will see you at home when issues arise if:

  • You live within our home visiting area
  • You have been assigned to our team
  • You are physically unable to make it to our clinic, but you wish to continue your care in your home setting

Not all patients followed in our clinic live in our home visiting area. If you live outside our home visitng area, we will help you arrange further support in the home with a local provider.

What to expect?

A date and approximate window of time for the visit at home is provided. Sometimes we will visit jointly with other team members from the community in the home.

What do I need?

  • If you have nurses visiting you from home care, we will need ask to see their binder.​
  • Medications
  • Support person​
  • Please tell us ahead of time if any special instructions regarding parking or entering the building, pets, smoking or hazards to be aware of.
