THP Perinatal Community Care Program

Trillium Health Partner’s (THP’s) Perinatal Community Care Program (PCCP) is an initiative designed to provide a wide range of services to support birthing people and their newborns during the first six to eight weeks, for individuals who gave birth at Trillium Health Partners.

This midwife-led program offers comprehensive perinatal care with a focus on postpartum care, support, and education to ensure the well-being of birthing people and their newborns. The program specifically aims to bridge the gap in postpartum care for marginalized and vulnerable populations, as well as those without access to a primary health care provider in the community. Referrals can be made by MRP, RN, SW or community agency.

  • Non-urgent Jaundice follow-up

  • Newborn weight checks
  • Telephone support
  • Postnatal clinical care for birthing parent
  • Resource and service navigation
    *Antepartum referrals are welcome, to facilitate care planning for postpartum care and parenting support.

    Home Visits

    Once the midwife receives a referral, they will review with the patient whether a home or clinic visit is possible.
