THP Perinatal Community Care Program
The
Perinatal Community Care Program (PCCP) provides added supports to families and babies in the community when they need it most.
Led by experienced midwives at
Trillium Health Partners, the PCCP offers comprehensive clinical care, support, and education to patients and newborns for up to 8 weeks after birth. Whether you're new to Canada, don't have access to a family doctor, or are navigating other challenges, the PCCP is here to help make the transition to life with your baby easier.
Our outpatient services are designed to bridge the gap between hospital and home, providing care that meets your needs in the right place, at the right time.
What We Do
The PCCP provides a wide range of services for THP patients during
the first six to eight weeks after birth, including:
- Postpartum clinical care for those without access to a primary care provider
- Newborn weight checks
- Infant feeding support
- Non-urgent jaundice follow-up
- Mental health support
- Contraception
- Telephone support
- Help finding resources and navigating community services
- Pregnancy consultations to help with postpartum planning
- Group education
*Please note - Our services are free of charge for patients with OHIP or Interim Federal Health. Translation services are available as needed.

Where Are We Located?
The PCCP provides services across the following locations:
Home Visits
**For patients with a Mississauga address only**
Home visits are available for those who are unable to access our hospital clinics. Home visit availability is determined on a case-by-case basis and will be prioritized based on need.
Mississauga Hospital - Family Care Centre
77 Queensway West, Suite 201
Located inside the Trillium Health Partners Family Care Centre, directly across the street from Mississauga Hospital. Parking is available at 77 Queensway West.

Located inside Valley House in the Maternal Fetal Medicine Clinic. Parking is available at Credit Valley Hospital.

Referral - If you could benefit from any of the services listed above, talk to your healthcare provider, nurse, social worker, or community support, and ask them for a referral to the PCCP.
- THP-affiliated providers: Submit referral in EPIC
