Trillium Health Partners understands that it is not always easy for seniors to leave their homes for appointments. That is why we have a dedicated Geriatric Medical Outreach team to provide comprehensive geriatric assessment services to you in your own home.
You may receive care from a team that may include a home visit from one or more of the following:
- Nurse Practitioner
- Occupational Therapist
- Social Worker
- Pharmacist
- Geriatrician
Recommendations may be provided to you with the goal of improving your health and overall quality of life. Your family doctor will be given a report and an appointment with a geriatrician will be arranged if needed.
