The Trillium Health Partners Transitional Age Youth (TAY) program is a physician-referred, voluntary, time limited treatment program for emerging adults aged 16-24 who have prominent symptoms of mood, anxiety, psychosis or borderline personality disorder.
The TAY team, which includes psychiatry, occupational therapy, and social work, focuses on providing stabilization and short-term treatment for emerging adults to ensure a successful transition from child and adolescent services to adult mental health services. Services are offered to both patients and their families or caregivers.
Our services:

- Individualized treatment plan with a focus on stabilization, treatment and transition to community or adult mental health services
- Specialized individual and group treatment services offered by an inter-professional team
